We provide comprehensive HIV care coordination in a confidential and supportive environment. Patients receive testing, education, and linkage to HIV specialty care, along with assistance accessing medications and insurance coverage.
Our team offers ongoing case management, appointment coordination, lab follow-up, and adherence support to promote viral suppression and long-term health.
Hepatitis C (HCV) Care & Treatment
We offer screening, evaluation, and treatment for Hepatitis C with a focus on timely access to care. Patients receive education about HCV, complete required lab work, and are guided through treatment initiation.
Our department assists with medication access, insurance navigation, and pharmacy coordination, while providing follow-up and support throughout treatment to promote cure and continuity of care.
PrEP & PEP
We provide confidential, patient-centered access to PrEP and PEP for HIV prevention. Patients are screened to determine eligibility and receive education on medication options, effectiveness, and expectations.
Required lab testing is completed prior to initiation. PEP is started as quickly as possible following a potential exposure due to its time-sensitive nature. Our team assists with medication access, insurance navigation, and pharmacy coordination to avoid delays in care.
Patients receive ongoing follow-up, lab monitoring, and adherence support. Individuals completing PEP are evaluated for transition to PrEP when appropriate to ensure continued HIV prevention.
